Weekly COVID-19 Updates

Dear Phoenix Center’s Residents, Families, Staff, and Visitors:

As of today, we have 0 active (+) employee case, quarantined at home and 2 active (+) resident case with proper care plan in place. Facility will continue to monitor all staff and residents for Covid-19 symptoms and will take any appropriate steps necessary if deemed to be symptomatic. The facility continues to follow all CDC and DOH recommendations and continues to report specific numbers to the DOH on a daily basis. Facility will continue to test staff and resident’s weekly.

We recommend having LIMITED visitation while we are in an outbreak to reduce the transmission of COVID-19. Please schedule with your social worker or front desk in advance to ensure visitation rooms are available to be used during your visit.


Due to the increase in cases for COVID-19 and the Omicron variant the facility is taking extra precautions. We recommend two visitors during visitation, and we kindly request NO children to come for visitation at this time.  Rapid test must be completed upon arrival and full PPE must be worn at all times.**

Failure to follow the policy may result in NO future visitation.

VACCINE UPDATE: Our Vaccine Clinic is held monthly. We are now able to vaccinate children from age 5 to 11 with Pfizer Vaccine. Social worker will be reaching out to families to get consent. See attached on information. We strongly encourage all family members to have all eligible residents vaccinated as it will allow more frequent visitation, socializing events such as more opportunity to be able to go inside the day room. Unit managers or your social worker will be reaching out to you to receive consent.

OTHER UPDATE: Dayroom’s activities are now open!

RENOVATION: We have completed the second-floor pediatric unit! We have started our first-floor pediatric renovation! Our goal is to ensure we are keeping our residents safe during construction. We have a safety plan that is available if anyone wishes to review it or if anyone has any questions, please be free to contact Administration.
